We don't believe in tying you up with unnecessary account creations or sign ups. We want this to be quick, easy and efficient for you. The health form takes around 10 minutes or so to complete. It's that simple.

Why do I need to do a health form?

 

As this is a prescription-only medication, we need to ensure you are suitable to use this medication. The health form is all about you, your history and your goals. Your information will NEVER be shared with any 3rd party. We only share your information with our partner pharmacies who source your medication.

Due to the current Ozempic shortage, we have joined with AJ Hill Aesthetics to offer a combined service that allows both companies to have access to each other's pharmacy partners. This has enabled us to offer our customers an uninterrupted supply each month. The health form and payment is carried out by AJ Hill Aesthetics, whilst one of our pharmacy partners will screen your health form. If suitable for treatment, they will source the medication for you and then the medication will be delivered to you on your selected delivery day.

What's the process?

 

• Once you have completed your health form, you can either pay for your order straight away or you can wait until we screen your form.

• Your form will be screened to ensure your suitability to use the medication.

• Once approved, if you have already paid, your medication will arrive on one of your selected delivery days. If you have not yet paid, you will be sent a manual email invoice via PayPal. You do not need a PayPal account; you can checkout as a Guest but you will still receive the same PayPal Buyer Protection that PayPal members get on every transaction. You will also have the option to use PayPal Pay In 3.
